Autumn Leaves provides a wide range of Clinical Support services that are provided to older adults to help them maintain their health, independence, and quality of life.
- Our care giver arranges regular check-ups, medication management, and treatment for chronic conditions under proper medical guidance.
- We provide medical care and assistance with daily activities for seniors who are unable to leave their homes.
- Hospitalisation assistance, Medicine delivery, Doctor and Healthcare personal home visit, all can be routed through us effectively.
- We also take care of medical instruments or ICU set up in the home if required.

We also offer medication management services to ensure that our residents are taking their medications as prescribed and that there are no adverse interactions between different medications.
In addition to medical care, we also offer rehabilitation and therapy services to support our residents’ physical and mental health. Our team of therapists and rehabilitation specialists provide individualised treatment plans designed to help our residents recover from injury or illness and regain their independence. We also do complimentary Blood tests included in our subscription plan in order to keep a tab on the health of our dear members.
We also offer end-of-life care services for individuals with terminal illnesses, ensuring that they receive compassionate and dignified care during their final days.
